Use this service to submit a social prescribing referral request.
You can use this service if you:
- are aged 16 or over
- are registered at the surgery
The Social Prescribing Service supports people who could benefit from social prescribing interventions. The service is non-medical and aims to look at the needs of a person holistically, regardless of what those needs might be, and find solutions to problems faced. Social Prescribing Link Workers will give you time to focus on what matters to you.
Social prescribing identifies opportunities for people to become more involved and active in their community by linking them to services, community activities and groups to help improve their health and well being.

- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
