Social Prescribing

The Social Prescribing Service supports people aged 16 and over who could benefit from social prescribing interventions. The service is non-medical and aims to look at the needs of a person holistically, regardless of what those needs might be, and find solutions to problems faced. Social Prescribing Link Wokers will give you time to focus on what matters to you.

Social prescribing identifies opportunities for people to become more involved and active in their community by linking them to services, community activities and groups to help improve their health and wellbeing.

Our Social Prescribers:

  • Usually meet people for the first time within their own home or out in the community
  • Spend quality time listening to people and finding out what matters to a person
  • Connect a person to appropriate services and groups, depending on the person’s needs
  • Help a person to identify their own strengths and assets
  • Support people to overcome barriers they may face to accessing support (for example, meeting a person at a group if their confidence is low or providing travel passes if transport is a barrier)
